LLCs are not recognized by IRS.
You don't really have to file anything but when you prepare the LLC for the state, include the new member as a partner and add his investments.

I spent a couple hours on the IRS's web site researching it.
The only thing I could find was in the Instructions for Schedule C that said:
"If you received cash of more than $10,000 in one or more related transaction in your trade or business, you may have to file Form 8300. For details, see Pub. 1544"I wasn't sure if the Business would be required to submit a form to the new member and the IRS indicating the amount of their investment.
